Position Summary
The Emergency Department (ED) Paramedic Intercept Program (PIP) is an ED based field response unit. The PIP Registered Paramedic (RP) is primarily responsible for responding to emergency calls for paramedic level care within the immediate and mutual aid catchment areas. While working in the field, the RP is responsible to adhere to the NH EMS protocols and any local option protocols selected by his/her Medical Director at Weeks. When not working in the field, the RP functions as a team member in the Emergency Department, working with the ED physicians, nurses and technicians. While in the ED, the RP is responsible to adhere to the hospitalaEUR(TM)s Standards of Patient Care and Policies/Procedures and function within his/her scope of practice. Pre-hospital NH EMS protocols do not apply within the confines of the ED - physician orders are required prior to interventions. The PIP Paramedic will also function as a member of the patient transfer team when acute inter-facility transfers are required.
Qualifications
Graduate of a Paramedic program.
Current licensure as a Nationally Registered Emergency Medical Technician-Paramedic.
Current licensure as an Emergency Medical Care Provider in the State of New Hampshire.
Valid driveraEUR(TM)s license and insurable by hospital insurance company.
Current BLS, ACLS, PALS and PHTLS certifications required prior to employment. Critical care course completion required within two years of employment.
MOAB required within one year.
Prior experience as a Paramedic for two years required.
Hospital experience preferred.
Excellent interpersonal skills, conflict management and time management skills required.
Basic computer systems knowledge.
Essential Functions of the Job
Completes the required clinical competencies for a Paramedic Intercept Program and Emergency Department Paramedic. Has competency checklist been completed with all competencies met?
Demonstrates current clinical competence and excellence in clinical practice as outlined in skills competency tool.
Responds to requests for pre-hospital paramedic services in a safe manner. Keeps crew, public, patient, vehicle and equipment safety in mind.
Accepts responsibility and accountability for his/her own clinical practice while functioning as a Paramedic in the PIP and ED. Adheres to the Standards of Practice in the ED (functioning strictly based upon physician orders) and when pre-hospital following NH EMS Protocols.
Tests all emergency equipment in the PIP vehicle and the ED at the start of the shift. Assures that all equipment is ready for any emergency situation that may arise.
Assures accurate charging for all supplies, medications and procedures performed in the field or the ED. Assists the EMS Coordinator or the ED RN to review and audit the patient charges if requested.
Accepts responsibility and accountability to identify and report Risk Management situations. Completes Occurrence Reports with all details documented. Notifies the EMS Coordinator, the Department Manager, Critical Care Services, and the Risk Manager when risk related situations arise.
Brings recommendations on new products and recommendations to reduce supply expenses to the EMS Coordinator and Department Manager as needed. Demonstrates a commitment to containing costs and efficient use of materials.
Assesses EMS patients immediately upon arrival at the scene. Assesses Emergency Department patients upon arrival to ED and at appropriate regular intervals. Obtain a bio-psycho-social history relevant to reason for presentation. Documents assessment findings and reports findings to the appropriate person.
Interprets rhythm strips accurately. While functioning in the ED, review all test results including (but not limited to) EKGs, Lab and X-ray and report findings to the ED physician.
In conjunction with the ED physician and ED RN, coordinates the in-hospital interdisciplinary plan of care.
Implements the plan of care including the administration of medications according to State and hospital formulary/policy. Monitors for desired effect as well as side effects and adverse effects. Documents findings.
Assists the physician and other providers in treatments and procedures (i.e. suturing, splinting). Implements physician orders within scope of practice for a Paramedic.
Reviews and updates treatment plan in collaboration with the pre-hospital team or the ED team, the family and the patient.
Input Physicians orders into Order Management system and carryout waive testing as needed. Processes other physician orders for diagnostic tests.
Documents all pertinent information on the pre-hospital PCR or the patientaEUR(TM)s medical record in an accurate, detailed and legible manner adhering to procedures for charting.
Transport patients to and from other departments as requested. Monitors patients and continues essential services (i.e. oxygen, IV therapy) during transport as indicated.
Ensure sensitivity toward patients and caregiver/family by eliciting patientaEUR(TM)s physical and emotional concerns, being a receptive listener, and providing an environment conducive to interviewing and the delivery of care.
Upon request, accompanies a patient on a paramedic level transfer from Weeks to another acute care facility. Ensures completion of NH Transfer form and obtains orders from physician prior to transfer.
Follows appropriate infection control practices and precautions as outlined in the hospital policies and procedures.
Supports patient care initiatives in all clinical departments by responding to Code Blue and Rapid Response Team calls in-house and performing IV starts in any department as requested.
Participates on CQI teams and implements new and ongoing quality improvement activities.
Accepts all other duties as assigned by the EMS Coordinator, ED physician, Department Manager, Critical Care or the Chief Nurse Executive.
