The Coordinated Entry Specialist provides non-judgmental supervision, leadership, and support services to Tyler Blain House shelter guests.
As the designated access point for Coordinated Entry, The Coordinated Entry Specialist will assist and engage those who are homeless and unsheltered or at imminent risk of homelessness with resolving their immediate housing crisis and connect them to the Homeless Intervention and Prevention (HIP) Outreach Worker for support with obtaining permanent housing.
Essential Functions &
Responsibilities:
N 10% Identify clients who are unsheltered homeless or at imminent risk of homelessness through reports/referrals form 211, other shelters, police, churches, town welfare officers, human service providers and others to resolve their immediate need and connect them with the outreach worker for their county.
E 10% Take appropriate action to deal with any homeless emergency situation; assist homeless individuals/families with obtaining transportation to shelter or services.
E 10% Conduct assessments and enter data into HMIS/EmpowOR database.
E 10% Develop and assist in implementing a Plan of Action for potential PATH, ESG, EHP, CTI & PSH clients utilizing PRE-CTI (in shelter) Case Management.
E 10% Maintain Personal, Organizational, Staff and Client confidentiality in a professional manner.
E 10% Maintain client records, shelter activity logs, staff communication log, client case-notes, and complete ALL reports/logs in a timely manner per shift and as assigned.
E 10% Ensure the facility is cleaned in a timely, thorough and efficient manner.
E 5% Establish and maintain positive, productive working relationships with partner organizations in order to serve as liaison for the homeless population.
This includes Physical and Mental health professionals, emergency services (Police, Fire or EMT) and State or Local government.
E 10% Assists shelter guests, as needed, in the performance of regular daily activities including managing needs and appointments as necessary and conduct regular rounds of facility to ensure wellness and safety of shelter guests.
E 5% Provide any necessary proactive crisis interventions.
Handle resident and house emergencies effectively and according to Crisis Management/Shelter Protocols.
E 5% Maintain proper boundaries while communicating and interacting with resident(s).
E 5% Receive, sort, inventory, and document in-kind goods received by the shelter.
Performance Measurements:
1.
100% 211referral requests on scheduled shift will be responded to no longer than 2 hours from time of request.
2.
The main Tyler Blain House phone will be answered within 3 rings and each call documented on the CE Master Client List.
3.
80% of housing crisis referrals received by CE specialists during your scheduled shift will result in a completed Housing Assessment, Diversion Assessment upon initial contact; 100% will be conducted by 2nd contact.
4.
80% of housing crisis referrals, that identify as unsheltered, received by CE specialists during your scheduled shift will result in a completed Vulnerability Assessment upon initial contact; 100% will be completed by 2nd contact.
5.
HMIS/EmpowOr data will be collected and entered no longer than 48 hrs following initial contact.
6.
95 % HMIS / EmpowOr Data entry will be free of errors and complete.
Knowledge, Skills and Abilities:
Experience One year to three years of similar or related experience.
Education (1) A two-year college degree or (2) completion of a specialized course of study at a business or trade school or (3) completion of specialized training courses conducted by equipment vendors or (4) job specific skills acquired through on-the-job training or apprenticeship program.
Interpersonal Skills Courtesy, tact, and diplomacy are essential elements of the job.
Work involves much personal contact with others inside and/or outside the organization for purposes of giving or obtaining information, building relationships, or soliciting cooperation.
Other Skills Experience working within diverse communities; acceptance of a variety of lifestyles, behaviors, and cultural and spiritual practices.
Understanding of harm reduction frameworks and how they apply to direct service Physical Experience working with homeless and/or at-risk of homelessness; ability to establish and Requirements maintain trusting professional relationships with house guests Work Environment Ability to remain calm and focused in potentially volatile situations.
Demonstrated de- escalation skills and experience working in crisis intervention.
